3 - return it to info@theprivatepracticeexperience.com.

You will receive a confirmation email once your payment has been processed and your seat has been reserved. 

In order to qualify for the Early Bird special ($449), this form must be submitted by 11:59PM ET on Webnesday, September 15, 2021. After 9/15, the registration fee returns to $499.
